We are happy to accept new patients residing or moving into our practice area.
Where appropriate please bring your completed registration form together with photographic proof of identity which will help us quickly obtain your medical records from your previous GP practice.
Although we accept the registration of patients outside our practice boundary, you will not be eligible for home visits. In addition to this, some community services will not treat you based on your location. You may be asked to change GP practice should you need to access these services.
Pre-Registration with the practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us.
Without ALL the above information, we will not be able to process your registration
All new patients will need to bring copies of any current medication and if a child under 16 years old we will need the full immunisation history - even if from abroad.
The practice operates an open list and any resident of the EU are entitled to services. Patients resident outside the EU will be required to present a visa valid for over 6 months. Copies will be made of this documentation this is to ensure entitlement is valid, so no existing services to entitled NHS patients will be compromised.
Appointment Reminder System
It is important that a mobile telephone number is given at registration so that the surgery may be able to text appointment reminders to you.
If previously registered with a GP in the UK please bring your medical card or complete a new registration form (GMS1). If new in to the country from an EU country please bring your passport, for non-EU residents please bring a valid visa and complete a new registration form (GMS1).
