ALL new patients will need to attend for a full health check with our healthcare assistant. You will need to bring to the appointment any current medication and if a child under 6 years old we will need the full immunisation history - even if from abroad.

The practice operates an open list and any resident of the EU are entitled to services. Patients resident outside the EU will be required to present a visa valid for over 6 months. Copies will be made of this documentation this is to ensure entitlement is valid, so no existiing services to entitled NHS patients will be compromised.

It is important that a mobile telephone number where available is given at registration so that the surgery may be able to text appointment reminders to you.

If previously rgistered with a GP in the UK please bring your medical card or complete a new registration form (GMS1). If new in to the country from an EU country please bring your passport, for non-EU residents please bring a valid visa and complete a new registration form (GMS1).

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Any children under 6 years old will need full immunisation history - even if not born in the UK. This information should be brought to the practice upon registration.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
